What we walked into.
The clinics had a legacy patient portal with a 2.1 App Store rating and a 23% no-show rate. They wanted something patients would actually use, without breaking their existing EHR workflow.
The approach.

Designed an app that feels like a messaging thread — no patient portal vocabulary, no nested menus.

Built a React Native app with Expo, native video via Daily, and a local-first queue so rural patients do not lose drafts on spotty LTE.

Wired an AI triage agent that drafts a structured intake note from the patient chat; doctors approve or edit in one tap.

Shipped behind a feature flag, rolled out clinic-by-clinic, tuned the prompts from real transcripts.
